The two cases are not: "I was removed by him" VS. "He refused my
request".
They are: "I REMOVED HIM" VS. "He refused my request".
So I still don't know how to distinguish...

to find the user in local roster by jid, if found, it means you were
removed by him, otherwise he refused your request.
